Revoke Consent in GetHelpOC

Due to GetHelpOC being a data sharing platform it is important that clients understand their rights and consent to having their information shared with GetHelpOC Participating Agencies. A client has the right to refuse or revoke their consent to sharing their information. Please follow the instructions below to find out what an agency should do in these situations.

 

Revocation of Consent

 

 

Uploading the Client Revocation Consent Form onto the Clients Profile

 

Step 1: Fill out the Client Revocation Consent Form on the GetHelpOC website. The form will be listed under Forms and Documents. Make sure the form contains signatures from both the client and the Organization Staff filling it out. 

 

 

Step 2:  Upload a completed copy of the Client Revocation of Consent Form to the client's profile page in GetHelpOC. To upload the file, you will have to go on the client's profile page.On the right hand lower bottom of the client profile page go to the "Files" section. Click on the "Add Files" button and you may upload the signed copy of the Client Revocation of Consent Form. 

 

 

Step 3: The agency/staff working with the client can email the form to revoke@UnitedWayOC.org to request support with anonymizing the client's profile. If the client revokes authorization at a later date, it will not affect information that has already been shared.
