Participating Agency Partners are able to make the necessary updates on the profile of the client.
Navigating the Client Profile Page
The client profile page has client demographic details that have been collected from either 211 or any participating partners. They will have access to edit and update any client demographic fields. You can click on the pencil icon on the fields to update the client record.
Client Page Base Navigation Flow Search 2.0: In the navigation flow, users are able to go over: consent, contact intake, general assessment, pre-screening internal 211 OC program and resource search.
Referral Section: Under the referrals section users can view all the referral history, status and the referral method.
Intake Section: This section shows general assessments for different programs mostly entered by the 211OC Connection Center team and may provide additional information of the client's situation and/or additional demographic information.
Care Team Members Section: Users can view the Care Team Members and the agency information of whom is working with the client.
Program Enrollments: Users can view all programs that the client is enrolled in.
Notes: Users can view all the notes across all participating agencies in the field.
Adding Notes: To add a note, you have to select the New Note on the top right of the client profile page.
Files: You can upload any files that will be visible to any participating partners. Files can be uploaded to either private or public.
Consent: In the consent section, the user can review the consent scrip with the client, The client can either accept or deny their consent. In the article Adding A New Client it discusses in further the steps to add a new client and it review the consent process.
Contact Intake: This section is 211's Contact Center team intake section.
General Assessment: In this section, the General Assessment is based on the SDOH & risk level. If someone comes into the network of care, we can do the assessment for their needs and an agency partner can complete the assessment again to track their progress from a critical to crisis situation to a stable or thriving state.
Select Pre-Screening: This section is primarily used for an additional 211 intake programs for our Contact Center team.
Resource Search: In the resource search section, a user is able to search for resources based off of the client's needs. In the article, How to Search for a Resource it will guide a user on how to search and send resources to the client.
